 Purpose of Collection                         

 As of this writing, there are no psychosocial stress assessment tool designed for Filipino first responders. It is the vision of this proposed android app that whenever a first responder volunteers for deployment, a series of interactive messages will be sent to his/her android phone before, during, and after deployment. His/her responses will be automatically analyzed to assess real time stress levels, triggering appropriate interventions, such as reminder to sleep, eat properly, stop his/her participation for a few days or indefinitely. Methodology: In the first 12 months, the stress assessment tool, android app and training modules will be designed, created, and pretested. This system will be deployed to disaster response organizations. All generated data in the next 12 months will be used to assess if the stress levels of first responders maybe managed with the Android app, as compared to pre-deployment.                                            

Types of Data collected
Among the types of Personal Data that this Application does not collect devices information such as contacts, camera, microphone, social media accounts and location. Complete details on each type of Personal Data collected are provided below.

Personal Data Collected
We collect the following personal information from you when you electronically register in Mi Salud Apps.
• Name
• Age
• Gender
• Birthday
• Institution
• Contact information, preferably Cell Phone Number, Email address   

Personal Data may be freely provided by the User, or, in case of Usage Data, collected automatically only upon registration when using this Application. All Data requested by this Application is necessary for this Application to provide its services. In cases of optional information not provided by the users, this will have no consequences to the availability or the functioning of the Service Users who are uncertain about which Personal Data is mandatory are welcome to contact the Owner.

Use of cookies – or other tracking tools by this Application or by the owners of third-party services used by this Application serves the purpose of providing the Service required by the User.. Users are responsible for any third-party Personal Data obtained, published or shared through this Application and confirm that they have the third party’s consent to provide the Data to the Owner.

Use The collected personal information is utilized solely for documentation and processing purposes within the Mi Salud Team and is not shared with any outside parties. Users enable the Mi Salud personnel to properly address the inquiries and requests, forward them to appropriate internal units for action and response, and provide [whom?–users?] with appropriate updates and advisories in an orderly and timely manner.

Protection Measures Only authorized Mi Salud personnel have access to these personal information, the exchange of which will be facilitated through email. They will be stored in a database for two years (after inquiries, requests are acted upon) after which digital files shall be anonymized.

Mode and place of processing the Data

Methods of processing The Owner takes appropriate security measures to prevent unauthorized access, disclosure, modification, or unauthorized destruction of the Data. The Data processing is carried out using computers and/or IT enabled tools, following organizational procedures and modes strictly related to the purposes indicated. In addition to the Owner, in some cases, the Data may be accessible to certain types of persons in charge, involved with the operation of this Application (Mi Salud personnel/Researcher and System Administration) appointed, if necessary, as Data Processors by the Owner. The updated list of these parties may be requested from the Owner at any time.

Legal basis of processing The Owner shall process personal data only with your consent and in accordance with the Data Privacy Act, its Implementing Rules and Regulations and other related issuances of the National Privacy Commission.

In any case, the Owner will gladly help to clarify the specific legal basis that applies to the processing of your personal data.

Place Your  personal data  shall be  processed at the Owner’s operating offices in the Philippines.

Retention time Personal Data shall be processed and stored for a period of three (3) years from the date of collection after which it shall be deleted. Consent from the personal data owner shall be obtained if the Owner needs to retain them longer for a legal purpose in accordance with the Data Privacy Act.

The purposes of processing The Data concerning the User is collected to allow the Owner to provide its Service, comply with its legal obligations, respond to enforcement requests, protect its rights and interests (or those of its Users or third parties), detect any malicious or fraudulent activity. The Mi Salud Application, with your consent,  may require any information from the devices such as Personal Data access, Analytics, Registration and authentication, Access to third-party accounts.

Device permissions for Personal Data access By default, these permissions must be granted by the User before the respective information can be accessed. Once the permission has been given, it can be revoked by the User at any time. In order to revoke these permissions, Users may refer to the device settings or contact the Owner for support at the contact details provided in the present document. The exact procedure for controlling app permissions may be dependent on the User’s device and software. Please note that revoking such permissions might impact the proper functioning of this Application. If User grants any of the permissions listed below, the respective Personal Data may be processed (i.e accessed to, modified or removed) by this Application.

Location permission This Application may collect and use, User location Data in order to provide location-based services.

Camera Used for accessing the camera or capturing images and video from the device.

The rights of Users Users have the right to:
• Withdraw their consent at any time.
• Object to the processing of their Data.
• Access their Data.
• Verify and seek rectification of their Data.
• Restrict the processing of their Data.
• Have their Personal Data deleted or otherwise removed.
• Receive their Data and have it transferred to another controller.
• Lodge a complaint before the National Privacy Commission and other government appropriate government agencies.

Legal action The User’s Personal Data may be used for legal purposes by the Owner in Court or in the stages leading to possible legal action arising from improper use of this Application or the related Services.

Additional information about User’s Personal Data In addition to the information contained in this privacy policy, this Application may provide the User with additional and contextual information concerning particular Services or the collection and processing of Personal Data upon request.

System logs and maintenance For operation and maintenance purposes, this Application and any third-party services may collect files that record interaction with this Application (System logs) use other Personal Data (such as the IP Address) for this purpose.

How “Do Not Track” requests are handled This Application does not support “Do Not Track” requests. To determine whether any of the third-party services it uses honor the “Do Not Track” requests, please read their privacy policies.

Changes to this privacy policy the Owner reserves the right to make changes to this privacy policy in which case, Users shall be notified via the contact information provided  to the Owner.
