Adding/Changing Subjects

PROCEDURE:

A. NEW STUDENTS

  1. Present an Admission slip issued by the Office of the Dean of Admission.
  2. Original Copy of Form 138 (Student’s Report Card) Certificate of Transfer Credential with Transcript of Records (TOR) – for evaluation purposes – (for Transferees/Shifters).
  3. Original Certificate of Good Moral Character duly certified by the School Principal or Guidance Counselor.
  4. Medical Certificate issued by the College Physician.
  5. Latest Photo/ID Picture – 1”x1” (4pcs).
  6. Birth Certificate (Certified True Copy) NSO.

B. OLD/RETURNING STUDENTS

  1. Present your Passbook duly cleared by your respective departments/institute.
  2. Medical Certificate issued by the College Physician.
  3. Latest Photo/ID pictures 1”x1” (4pcs).
  4. Class cards, 2nd semester 2016-2017.
  5. Evaluation of subjects, grades and units taken.
